Medical Services
 

Mrs. Queberg is available school hours Monday through Friday.  Students needing her services should report to the Information Desk or contact the high school office at 330.725.8461

When should your child stay home from school?

  • A temperature of 100 degrees or higher

  • Any vomiting and/or diarrhea within the last 24 hours

  • Eye symptoms such as redness, itching and/or unusual white or yellow drainage

  • Has lice or nits; students may return to school after they have been properly treated, are nit-free and rechecked by the nurse or health aid

The Medina County Career Center medical services department provides medical services to students and staff which include:

  • First Aid

  • Health Education

  • Community Resources

  • Referrals

  • Communicable disease reporting to Medina County Health Department

FORMS & RESOURCES  

Asthma Action Plan
Complete and submit action plan relating to asthma to the high school nurse.

Asthma Inhaler
A completed form must be provided to the school principal and/or nurse before the student may possess and use an asthma inhaler in school.

Bee/Wasp Sting Action Plan
Complete and submit action plan for symptoms and medication requirements relating to bee/wasp stings to the high school nurse.

Bee/Wasp Sting Allergy Record
Describes reactions to bee/wasp stings. To be submitted to the high school nurse.

Diabetes Management Plan
Complete and submit action plan relating to diabetes to the high school nurse.

Diabetic Care Plan

Epinephrine Auto-Injector
A completed form must be provided to the school principal and/or nurse before the student may possess and use an Epinephrine Auto-Injector to treat Anaphylaxis in school.

Food Allergy Action Plan
Complete and submit action plan for symptoms and medication requirements relating to food allergies to the high school nurse.

Immunization Summary for Student School Attendance

Medina County Health  Department

Over the Counter Medication Authorization
Completed and must be signed by Parent/Guardian and returned to high school nurse.

Parents Consent for Blood Donation

Prescription Medication Authorization
Completed and must be signed by Parent/Guardian and Physician and returned to high school nurse.

Seizure Action Plan
Complete this form to better assist our staff if a seizure should occur during school hours.

 Mrs. Swartz
Mrs. Swartz joined the Medina County Career Center in 2006 as the Student Assistance Counselor. As a student advocate, she provides support and resources to MCCC students. Mrs. Swartz holds a Masters Degree in Education and is licensed by The State of Ohio as a Licensed Professional Clinical Counselor and a Licensed Marriage and Family Therapist.
